Disable TOTP for login from certain IPs or subnets

There is no need of MFA when login is done from certain IPs (LAN for example) or subnets, so it should be possible to avoid MFA in these situations.

    disable TOTP for internal IP range is a common request also here. Like Microsoft and other player

    Alternatively to IP subnets, a HTTP request header could be used to identify external requests.

    Such a HTTP request header could be provided by a HTTP Reverse-Proxy/Load Balancer.

    For example, this is how the Microsoft ADFS IdP differenciates between internal and external requests.
